What is Better Birth Outcomes – Navigation?
Better Birth Outcomes – Navigation (BBO-N) is a program that helps pregnant, and parenting clients understand the importance of a medical home and connects them to a variety of medical, social, and other benefits and supports in their community. The BBO-N program provides these services to dyads (pregnant and postpartum individuals and their infants) throughout pregnancy and the first six months after birth to Illinois residents who meet the eligibility criteria for the program.
One on one assistance
By providing direct, 1:1 assistance to connect and engage with desired services
Male Involvement
Helps the client meet their family’s health and wellness goals.
Nutrition Support
Providing educational support on safe sleep for infants, nutritional support for you and your baby, and support in reaching early development milestones.
Resource connections
Connecting you with in-network medical providers and with behavioral health and community services.
Health management during pregnancy
Managing health during and after pregnancy.
Transportation Assistance
Providing transportation, housing, food assistance, and more.
Eligibility
BBO-N provides services to those who meet one of the following criteria:
- Must live in within any of these zip codes: 60609, 60621, 60629, 60632, 60636, or 60638
- Have an income below 200% federal poverty level.
